So I have a 5IV male Bagon with 31/31/31/x/31/31 holding a destiny knot with my Adamant Axew with Everstone (terrible IVs) when I go to the IV checker, there's only decent IVs. Am I doing something wrong?

The destiny knot pick 5 random IV's between axew and bagon. For example he picks 
Bagon: Attack and Defense
Axew: Sp. attack, Sp. Defense and speed

Making an Axew: x/31/31/x/x/x

I suggest to you to trade your axew in the day care, to the breeded axew, to have more chances to get a 5IV's Axew.

And you do that every single time when you get an Axew with better Iv's.

Ok this is what you really want to do in this situation. To get a decent offspring from those two, get a good ditto with destiny knot, put the Axew with the everstone in the day care with the ditto. Breed a good female axew out. then use the good Azew baby with the bagon. Bam your done
